Our transitional care program supports individuals moving from hospital to home, between living environments, or into adulthood.
Our complex care teams receive specialist training to support individuals with high medical needs. This includes expertise in tracheostomy care, PEG feeding, seizure management, and vital sign monitoring. Staff are also trained in safe moving and handling techniques and can deliver tailored physical therapy to promote comfort, mobility, and wellbeing.
